Integrated care, hospital treatment and domestic nursing care
In "integrated care" patients are treated in cross-sectoral respectively interdisciplinary structures employing quality assurance. Doctors, specialists, hospitals, prevention and rehabilitation facilities and other health care providers can cooperate and ensure the necessary knowledge exchange. For this purpose health insurance institutions conclude corresponding agreements with service providers.
The health insurance funds can grant their insured persons a bonus for participation in an integrated form of care.
Since the GKV-WSG 2007 ("Gesetzliche Krankenkassen-Wettbewerbsstärkungsgesetz" - Statutory Health Insurance Competition Reinforcement Law) integrated care networks should increasingly aim at the nationwide treatment of widespread diseases, such as diabetes mellitus or intervertebral disc diseases, or treat all diseases of a patient in a network.
To improve the overall medical and nursing care situation of elderly and infirm patients, the nursing care insurance is also included in integrated care.
