• Data without additional payments by the insured according to § 28 Abs. 4 SGB V (treatment fee, "Praxisgebühr").
• Because of the place of residence principle introduced on January, 1, 2003, only the data for "Germany total" are comparable with the previous years starting from 2003.
• Excluding direct settlement cases in the field of orthodontics (2. half of 1997 and year 1998) as well as in the field of dentures (years 1998 and 1999).
• In the field of prosthetics the fixed allowance system was established in 2005. Related to the modified account classification the supply, which is not defined as standard supply ("Regelleistung") will no longer be billed to the Regional Dental Authorities but directly to the patients.
